Insurance Correspondence
As a courtesy, we have provided sample letters of correspondence to help you obtain a prior authorization for testing or to appeal insurance partial payment or denial of your claim. It is important that you contact our billing department if you are filing an appeal; we can assist with this process. It is also important that you keep a record of your appeal process including all correspondence you have with your insurance company and any documents such as an explanation of benefits (EOB) or a letter of medical necessity from your doctor.
